Effective Date
The date coverage under the insurance policy commences.
Eligible Dependent
A dependent of a covered person who satisfies the requirements specified in the contract to qualify for coverage.
Eligible Expenses
The lower of the reasonable and customary charges and the actual heath care costs incurred, that are covered under a health plan.
Employee Assistance Program (EAP)
A mental health counseling service offered to employees. Employees do not normally have to pay for services received through an Employee Assistance Program.
Employee Benefit Program
The combination of benefits offered to employees in addition to normal salary or wages. Employee benefits may include health insurance, dental insurance, life insurance, disability insurance, retirement plan contributions, stock options, and other benefits.
Endorsement
An amendment or modification to an insurance policy used to add, exclude, or amend coverage. Also known as a Rider.
Exclusion
A cause, condition, service or supply that is not covered under an insurance policy.
Explanation of Benefits (EOB)
A statement issued to an insured by an insurance company detailing the services provided, costs incurred, amount of eligible expenses, and amount covered by the insurance company. The insured is required to pay the outstanding balance, if any.
